Resident Resources

We provide Wisconsin nursing home neglect resources for families of residents.  We want families to have as much information as possible to make a safe and educated choice about the nursing home their loved one is going to call home.  You should use the Checklist we have below when visiting any facility.  Be an informed consumer.  We also provide resources and links to information you need if a resident has been harmed due to neglect and/or abuse.

We recommend an excellent book, "Consumer Guide to Long Term Care", by Gary R. Ilminen, RN.  You can order a copy at the University of Wisconsin Press at http://www.wisc.edu/wisconsinpress/books/3075.htm.  It is described as "A must-read for adult children of the elderly and anyone working in long-term care . . . Embrace Care America is a geriatric care management service that assists individuals and families to determine and select senior resources and services. The range of services includes comprehensive assessments, recommendations in a personalized care plan, implementation of the care plan and on-going monitoring and involvement. Nursing Home Checklist PDF File is a checklist for families to use when touring a nursing home.  It addresses the important areas for families to evaluate.

Resident Concern / Complaint Form is a form for families to document any complaint, grievance or concern about a loved one's care.  This complaint form can be turned over to the nursing home for the nursing home to address.  It also documents the family's concern(s) and shows the effort to improve the care.
